    HKBN x Evercare's Home Care Service Plans are for Hong Kong Broadband (HKBN) customers and is provided by Evercare Health Limited.

  • FAQ

    1. How can I activate my plan?

    • Customers must first purchase their desired plan through HKBN’s website or app. You’ll then receive an activation code via SMS or email. Next, enter the activation code along with your personal details in the above form. After successful verification, you’ll receive an email within 1-2 days with instructions to start booking service.

    2. How can I book services?

    • You can book services by submitting the "Evercare Service Agreement Form".  The form must be submitted within 90 days of receiving the activation code. 

    3. How far in advance should I book my service?

    • Services must be booked at least 7 days in advance. If you need to book with less notice, an urgent matching fee of HKD $500 applies.

    4. Can I rollover unused services to the next month?

    • Unused services will be forfeited and cannot be rolled over to the next month. 

    5. What are the Case Manager Consultation and Nutritionist Consultation, and when can I use them?

    • The premium plans includes two consultations with a Case Manager and two with a nutritionist. A Case Manager Consultation involves personalised support and guidance on the most suitable ways to arrange care for based on the patient's condition. A Nutritionist Consultation will give individuals an understanding of recommended lifestyle changes and dietary modifications. These consultations can be scheduled at any time during the 12-month contract period, based on availability.
